The inspector’s account

At 7:30am on 06/10/2025, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct a case management visit pertaining to two incident reports submitted to Community Care Licensing (CCLD) on 06/05/2025, from this facility and the Day Program (405801528) pertaining to Resident 1 (R1) having a black eye. LPA was accompanied by Tri Counties Regional Center (TCRC), Quality Assurance, Wesley Marking (QA). QA and LPA met with DSP 1, Edward Holliday (S1), announced who they are and the reason for the visit.

QA and LPA conducted observations and interviews. LPA requested documentation. LPA observation R1 upon entrance, R1 was grabbing ears and face with both hands (Stemming). R1 non-verbally declined to be interviewed. QA and LPA observed night shift logs from 06/06/2025 through 06/10/2025 indicating R1 sleeping and position with no abnormal note entries. LPA noted that on 06/06/2025 facility took R1 to urgent care to assess eye bruise and Urgent Care discharge paperwork indicated general eye trauma. On 06/10/2025, QA and LPA observed bags under both eyes. LPA interviewed S1, who stated that R1 grabs ears and eyes when stemming. S1 stated that they believed that R1's eye bruises are a result of R1 sleeping position. QA and LPA discussed the nature of the eye bruises on R1 have changed tone from dark to light, dark to light over the past 8 days, as observed in person and through photos. LPA was not able to determine any findings at this time.
