Facility Evaluation Report
At 7:45am on 11/19/2025, Licensing Program Analyst (LPA) Jeffries arrived to the facility announced to conduct the facility annual inspection. LPA met with facility Administrator, Clinton Cadle announced who he is and the reason for the visit.
This facility is a 5 bedroom, 2 bathroom, with kitchen, dinging room, living room, laundry room, and garage. There is a fenced back yard with seating and umbrella for clients to have visitors and activities. All clients rooms are single occupancy and the 5th bedroom is designated as a staff room. All client rooms have furniture, linins, chair, stand and lighting as required by regulations. LPA noted that both bathrooms are available to all clients and have paper towels and liquid soap. LPA observed at least two days of perishable foods and at least seven days of non perishable foods on hand for 4 clients and staff. LPA noted that medication are kept in the dining rooms area in a locked cabinet and a complete first aide kits is also located in the locked cabinet. LPA noted that there is a fire extinguisher that is primed and in the green located under the locked cabinet. LPA noted that the facility is clean and in good repair with all door ways, hallways, and exits free of obstacles. LPA noted that emergency water supplies and Personal Protective Supplies are located in the garage. Administrator tested carbon monoxide detector and LPA noted dual smoke /carbon monoxide detectors located throughout the facility. LPA noted that there were no violations or citations issued as a result of the physical inspection.
Administrator and LPA conducted a full review of the annual care tools. LPA noted that Staff and Client files were reviewed. LPA reviewed Emergency Care Plan, Infection Control Plan and conducted a cursory review of Client Centrally Stored Medication Records. LPA noted that there were no violations or citations issued as a result of the care tools and file review of the annual facility inspection. LPA noted that this annual facility inspection there were no violations or citations.
