The inspector’s account

At 9:19 am on 11/4/2025, Licensing Program Analyst (LPA) Rankin arrived to the facility, unannounced to conduct a case management final annual facility inspection. LPA met with Administrator, Jamie Staine, and provided the reason for the visit.

Physical Plant & Environment Safety: Facility has 4 bedrooms, and 3 bathrooms which are used for clients. LPA re-toured areas of the facility which included gates, garage, laundry room and kitchen. Disinfectant and cleaning solutions are inaccessible to clients in care and locked under the kitchen sink and in garage. On initial visit LPA took an image of the exit door from the garage to the back yard where a 2x4 wood bar had been secured, LPA asked why the bar was there, Administrator stated that one of their clients has wandering/elopement behaviors. The use of barring a door to prevent wandering is not allowable, the concern of staffing being sufficient to supervise and care for the resident was addressed, pursuant to Title 22, California Code of Regulations, the following deficiencies are cited (refer to LIC809-D). During initial visit bar was removed prior to LPA leaving, upon return, bar was still gone. Administrator and staff interviewed stated bar had been there for years, under prior administrator. It was discussed that the concerns of the client wandering have been addressed, there is a 1:1 staff assigned, the client also has a watch that alerts parent and administrator if client leaves, additionally the sheriff department has a band on the client that alerts the sheriff’s office if the client leaves the facility.
