Facility Evaluation Report

Licensing Program Analyst (LPA) Rankin conducted a case management closure visit to the facility above. LPA met with the Licensee Aprilyn Soriano and explained the purpose of the visit.

LPA did a walk thorough of the home. The home is empty with no residents at the facility. Licensee has 1 other Residential Care for the Elderly facility, resident signed notice that they agreed to transfer to the licensee's other facility. The last resident moved out on 3/15/26.

All evidence of residents personal items has been removed. The closure date of 3/25/26 is the official date no longer in business.

Licensee surrendered the license to the LPA.
