The inspector’s account

Licensing Program Analyst (LPA) Melisa Rankin arrived at 8:40 a.m. to conduct a 1-year required annual visit. LPA met with Aprilyn Soriano, Administrator. Facility is scheduled to be closed but currently has 1 resident pending the 60 day closure eviction.

A tour of the inside and outside of the facility was conducted. The following was inspected and noted during the annual visit:

Physical Plant & Environment Safety: LPA toured resident rooms, common areas, bathrooms, halls, outside areas and observed that all areas were tidy and free of odor. Toilet, hand washing and bathing facilities are operational and secured grab bars are present. The pathways are clear of any obstructions. The facility has a fenced backyard for client use with plenty of shade. The fire extinguisher was last charged and inspected on 10/28/25. The facility has smoke and carbon monoxide detectors that were tested. LPA toured all rooms, they are empty of any personal items, only room with personal items is the one with the remaining resident.

Operational Requirements: The facility has a current plan of operation on file with the department. The Facility is operating in compliance with the granted fire clearance. Facility currently has 1 non-ambulatory resident.

Staffing, Personnel Records & Training: The facility currently employs 2 full time staff and 1 administrator. Files reviewed had current 1st Aid/CPR and all other required hiring information. Staff have annual training completed for various subjects/topics and hours for 2025.
