Facility Evaluation Report
Licensing Program Analyst (LPA) Melisa Rankin arrived at 11:39 a.m. to conduct a 1-year required annual visit. LPA met with Aprilyn Soriano, Administrator.
A tour of the inside and outside of the facility was conducted. The following was inspected and noted during the annual visit:
Physical Plant & Environment Safety: Facility has 6 bedrooms, 2 bathrooms, 4 bedrooms are used for clients. LPA toured client rooms and observed that rooms were tidy and free of odor. The lighting and lamps are sufficient for the use of the facility and for client comfort and safety. Toilet, hand washing and bathing facilities are operational and secure grab bars are present. The showers have non-skid mats/flooring. The pathways are clear of any obstructions. Disinfectant and cleaning solutions are inaccessible to clients in care and locked under the kitchen sink and in garage. There is sufficient space inside and outside for activities and visiting. There is a fenced backyard for client use with plenty of shade. The facility has telephone and internet service for client use. The fire extinguishers are charged and an inspection is scheduled for tomorrow. Smoke and carbon monoxide detectors were tested and working at the time of visit.
Operational Requirements: The facility has a current plan of operation on file with the department. Facility is operating in compliance with the granted fire clearance. The fire clearance is granted for 4 non-ambulatory. The facility currently has 4 clients.
