Facility Evaluation Report
Licensing Program Analyst (LPA) Kristin Kontilis conducted an unannounced required Annual Inspection at the above-named facility. LPA met with Kalli Gedwed, Program Coordinator and Christine Pham, Medical Facilitator and explained the purpose of the visit. Program Administrator Valeria Ramirez accompanied LPA during the inspection. At the time of arrival, there were two staff on duty and eleven clients present. Nick Elia, Program Administrator was not available at the time of the inspection.
There are currently twelve (12) clients residing in the two-story facility that is home to clients with a mental illness diagnosis. The facility contracts with Santa Barbara County Behavioral Wellness.
A tour of the physical environment and accommodations were assessed, and the following was noted: LPA observed the required posting of the complaint poster and Resident’s Rights. LPA inspected the facility for fire safety, personal accommodations, and food service.
The physical environment was checked for cleanliness and condition. Walls, windows, ceilings, doors, floors and floor coverings were checked. At approximately 2:46 pm, LPA observed the back house bathroom linoleum is in disrepair. The shower wall has paint peeling on the outside. The transition strip in the entrance to the bathroom is decayed. LPA observed a "musty" smell in Bedrooms 1 and 3. Fire inspection was current as of 5/26/2025. LPA observed the carbon monoxide and smoke alarms are in good working order.
The kitchen area was sufficiently stocked with two-day perishables and seven days of non-perishables. Frozen foods are properly wrapped and stored appropriately. LPA observed the kitchen cabinets, refrigerator, stove, and counters are clean.
