Facility Evaluation Report

Licensing Program Analyst (LPA) Kristin Kontilis conducted a Case Management – Other visit at the above-named facility.

On 1/26/2026, Administrator Rhoda Demonteverde notified the Woodland Hills Regional Office (RO) of the facility closure and all residents have moved out of the facility. During today’s visit, LPA observed facility to be vacant inside and outside and verified that no care and supervision are being provided. LPA observed a “For Sale” sign posted on the front lawn of the facility. Licensee Norma Demonteverde will surrender the license directly to the LPA’s attention at the Goleta Regional Office via hand delivery or U.S.P.S.

Closure of the facility is confirmed as of this visit. Exit interview conducted. A copy of the report has been emailed to Licensee.
