The inspector’s account

Administrator stated that C1 got upset and began arguing with Administrator and eventually ended up going to the C1’s room. Administrator denied yelling at C1 during the incident. Administrator stated that clients can refuse to go on outings and C1 has refused to go with specific staff when taking clients on outings but did not recall any specific outing for dinner that C1 did not go on or refuse to go. On 02/12/2026, LPA conducted an interview with Staff 1 (S1) who was working on 02/06/2026, S1 stated that C1 was upset about the lack of attention C1 was getting from a male staff prior to the cake mix incident. S1 stated that Administrator did not raise voice with C1 and only told C1 that the cake mix was for the facility’s Super Bowl party and provided C1 with alternative food choices. S1 stated that C1 got further upset and went to C1’s room. S1 stated that C1 refused to go on the outing often because of specific staff not working during the outings. S1 stated that C1 has never been refused to go on outing by staff. S1 stated that C1 chooses to refuse to go on outings depending on which staff are working. S1 stated that staff will remain at the house when C1 refused to go on outing. On 02/12/2026, LPA attempted to interview C1, C1 shook head in side to side in the negative when asked if staff or administrator has ever yelled at C1, C1 was reluctant to answer additional questions and refused to answer any other questions during the interview. On 02/12/2026, LPA conducted an interview with S2, S2 stated that they have never heard or seen the Administrator yell as clients in care. S2 stated that staff do not prevent any clients from going on outings. S2 stated that C1 has refused to go on outings in the past. On 02/12/2026, LPA interviewed C2, C2 stated that Administrator does not yell at C2 or other clients. C2 stated that they go on outings when they (Clients) want to go. On 02/12/2026, LPA collected staff training documentation and noted that all staff are up to date on regulated training hours. At this time there is not enough evidence to support the allegations of, “Staff do not treat resident equally to other residents in care.” And “Staff yell at resident in care.” And both are unsubstantiated at this time.
