The inspector’s account

At 8:45am on 08/14/2025, Licensing Program Analyst (LPA) Jeffries arrived unannounced to conduct this facility annual inspection. LPA met with Administrator, Charles Barns, announced who he is and the reason for the visit.

This facility is a vender of Tri Counties Regional Center (TCRC) level 4i home. LPA contacted TCRC QA, Miguel Magana for update with no issues on client status during the inspection, as there is a state conservatorship authorization on TCRC's level 4i designation.

This facility has 5 bedroom and 3 bathrooms. Master-bedrooms is designated for staff with on suite bathroom. The other 4 bedrooms are single occupancy client bedrooms. Clients share the other 2 bathrooms. LPA noted that bedrooms are furnished per licensing regulations with proper linin, bedding, storage, seating and lighting. LPA observed all bathrooms to have paper towels, liquid soap, and non slip mats in the shower/tub. LPA noted that the water temperature throughout the facility was within regulation temperatures (105*-120*(f)). LPA noted the facility has a large family room, a large dining room, den, medication room and kitchen. Medications are locked in a cabinet in the medication room. The facility has a complete first aide kit located under kitchen sink. LPA noted that there is at least 2 days of perishable foods. and at least 7 days of non perishable foods on hand for 4 clients and staff. LPA noted that the facility has outdoor space in front and back yard. The back yard is currently under construction to add a hot tub. LPA noted that the construction currently has a large hole and deck exposed, administrator will update construction progress to LPA as needed. LPA noted that the facility fire extinguisher is primed and in the green located in the medication room. LPA noted that the facility has duel carbon monoxide detectors/smoke detectors hardwired in each client room and throughout the facility. LPA noted that the facility is clean and in good repair. LPA conducted a full physical tour and a full review of annual care tools modules with no violations and no citation issued. Exit interview, report read, and report provided.
