Facility Evaluation Report

At 8:00am on 12/15/2025, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct the facility annual inspection. LPA met with Facility Administrator, Sylvia Rae Solorio, announced who he is and the reason for the visit. LPA noted that this facilities last annual inspection was conducted on 01/07/2025.

LPA noted that this facility has four bedrooms, two living rooms, dinette, kitchen, laundry room and a butlers pantry where a locked cabinet where medications are stored. There is a fenced back yard, with tables and chairs and a covered patio with ample room for client activities and visitors. Four of the bedrooms are single client occupancy and two bathrooms are open to all clients and staff. Both bathrooms have liquid soap and paper towels. LPA observed bedrooms to be properly furnished with storage, chair and lighting, as well as proper linin per regulations. LPA noted that the was at least two days of perishable foods and at least seven days of non perishable foods on hand for four clients and staff. LPA noted that there is a working fire extinguisher primed and in the green reading located in the facility front door. LPA tested water temperature on the facility to be within regulation standards of 105*-120*(f). LPA noted that there is a full first aide kit located in the closet on the south side of the facility. LPA noted that smoke detectors are dual smoke and carbon monoxide detectors and were tested to be working. LPA noted that the facility is clean and in good repair and all passageways and exits were free of obstacles. LPA reviewed facility Emergency Disaster Plan, Infection Control Plan, Staff files, Client Files, and a sample of Centrally Stored Medication Records.

Administrator and LPA conducted a full review of the annual care tools modules. LPA noted that there were no violation or citations as a result of the annual care tools modules review. LPA noted that there are no violations or citation as a result of this annual inspection.
