Licensing Program Analyst (LPA) Olson conducted an unannounced on-site visit to the facility in regards to an incident that occurred on 7/4/2022. LPA met with Administrator Chris Patton and explained the purpose of the visit.
LPA obtained documents regarding Client 1 (C1) and interviewed multiple staff and Administrator.
At this time, further investigation is needed. LPA will follow up at a later date to continue the investigation.
Exit interview conducted and a copy of the report emailed to the administrator/licensee.
