Facility Evaluation Report
At 1:30pm on 04/07/2026, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility on a case management visit as a result of discovering Staff 1 (S1) who has an Exclusion identified in Guardian and LIS while working at this facility. This was discovered during an annual visit at a separate facility under the same Licensee (PER ANKH LIFE SKILLS, INC. .) LPA arrived to conduct a case management visit to ensure that S1 is not currently working at the facility. On 04/07/2025, prior to the LPA arriving on this case management visit, facility Administrator suspended S1 from working until exclusion can be clarified through Guardian background clearance process. LPA noted that S1 status in Guardian showed "In Progress" and not cleared and the staff therefore should have not been working at the facility until cleared to work. LPA i ssued a citation for 82019(e)(2): (e) Prior to working, residing or volunteering in a licensed day program, all individuals subject to a criminal record review…shall do the following: (2) Obtain a California clearance or a criminal record exemption as required by the Department. At this time a citation and civil penalty is assessed for S1 working at the facility.
