The inspector’s account

At 7:30am on 04/07/2026, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct the facility's annual inspection. LPA noted that there was no answer at the door, LPA called Administrator, Arnold Hernandez and noted that Clients leave for program just before 7:00am. Administrator stated that he was in route to facility to review annual inspection with LPA. Administrator arrived to the facility at 8:45am.

Administrator and LPA conducted a full facility tour. This facility has 4 bedrooms, and 2 bathrooms, two living rooms, kitchen and dining room. The back yard had a table chairs and pergola for shade. All bedrooms are set up to regulation standards. LPA noted that there are smoke detectors throughout the facility that are hard wired and a carbon monoxide detector all functioning normal. LPA observed a fire extinguishers primed in the green. LPA noted that there is a full 1st aide kit per regulations located in the kitchen closet. Medications are locked and secured in a medication cabinet art in the staff office. LPA noted that all exits and passageways are free and clear of obstacles and debris. LPA noted that the home is in brand new condition and no violations were observed during the annual physical facility tour. LPA conducted a review of Client and Staff files, Emergency Disaster Plan, Infection Control Plan and Emergency Drills. LPA reviewed Administrator Certificate and current staff associations. LPA discovered that Staff 1 listed on staff schedule not having obtained a California clearance or a criminal record exemption as required by the Department (80019(e)(2). A citation and civil penalty was accessed.

Administrator and LPA conducted a full review of annual care tools. At this time there is one citation as result of this annual facility inspection.
