Facility Evaluation Report

At 1:30pm on February 26, 2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived unannounced to the facility to conduct a facility closure visit. LPA met with backup administrator Junsik Kim announced who he was and the reason for the visit. Licensee/Administrator Young Won emailed LPA official notice of closure on February 20, 2026.

LPA conducted a full tour of facility and facility property. LPA noted all client belongings were gone and no indication of clients residing in the facility. The physical facility license was not present at the facility, backup administrator will attempt to find it and LPA will collect it during an annual visit at one of the other facilities owned by licensee. LPA noted the facility appeared to be absent of all clients at this visit and the backup administrator indicated that the last time a client resided at the facility was April 2025.
