The inspector’s account

At 07:40am, on 07/10/2025, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA met with Administrator Amy Hicks, announced who he was and the reason for the visit.

Licensee and LPA conducted a full tour of the facility. This facility is a residential home with three dual occupancy client bedrooms, two full bathrooms (one is on-suite to a client bedroom and one is for shared use). There is a kitchen with dining space and living room. Access to the laundry room and garage is through a locked door for resident safety. LPA noted that the backyard has seating and shade for clients. LPA noted fresh fruit and snacks in the kitchen for clients to enjoy freely. At 11:58am LPA observed a bottle of Great Value spray bleach, a aerosol can of Febreze pet odor fighter fresh scent, and a can of Lysol Air Sanitizer in the hallway closet accessible to clients. The facility has battery operated smoke detectors in each room that are all working, the carbon monoxide detector is in the hallway and functioning normally. LPA observed fire extinguishers throughout the facility tagged current and in th e green compression range, serviced on 11/05/2024. LPA tested facility hot water at 106.7*(f), within regulation temperatures 105*- 120* (f). LPA observed at least 2 - days of perishable and at least 7 - days of nonperishable foods. While touring the kitchen LPA noted over 15 canned and boxed foods to be of 1-3 years past the dated shelf life . LPA noted the facility to be free of obstructions in hallways, doorways or exits. Medications are locked in a cabinet in the hallway. Staff and client files are locked in the dining room. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records, finding no violations. LPA conducted a staff and resident file review.
