The inspector’s account

At 9:50am, on February 17, 2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA met with Administrator Traci Hollinger and the Program Coordinator(PC), Lote Rouse, announced who he was and the reason for the visit.

Administrator, PC, and LPA conducted a physical tour of the facility. This facility has a main lobby, conference room, six classrooms, kitchen, two storage rooms and two public bathrooms. There is also a sick room with a shower. Lunches are brought from home by the clients and assisted as needed by staff. There is a supply of food and snacks on hand for clients who did not come to the program with lunch. The facility has wired smoke detectors tested by Great Western Alarm on 6/12/2025 and the carbon monoxide detector is in the hallway and functioning normally. LPA observed fire extinguishers throughout the facility tagged current and in the green compression range, serviced on 7/14/2025. LPA noted that the facility is clean and in good repair with no obstructions in hallways, doorways or exits. At time of inspection there are no clients serviced that require medication management, but the facility has a plan in place to assist residents with self-administration and storage of medications. LPA conducted a staff and resident file review. LPA noted that there is an administration office attached to the facility on the north side of the facility and occupational program on the south side of the facility, both administration and occupational program have inside access to the facility but are not part of the licensed Adult Day Program. LPA did not note any visible violations on the cursory tour.

LPA, PC, and Administrator conducted a review of the annual care tool modules. There were no deficiencies cited at this time.
