Facility Evaluation Report
At 12:00pm, on 5/21/2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the post licensing facility inspection after the admission of the facilities first resident. LPA met with lead staff Joselito Efondo, announced who he was and the reason for the visit. At 12:06pm LPA called Licensee/Administrator Amelita Aquino to notify them of the reason for the visit.
Joselito and LPA conducted a full tour of the facility. This facility is a single story residential home with four resident bedrooms (two are dual occupancy) and three full bathrooms (one is en suite to resident room #4, one is a jack-and-jill bathroom connecting resident room #3 and the live-in staff bedroom and one is for public use). A live-in staff bedroom is located near the laundry room. There is a living room, family room, dining space, and kitchen. The family room has a self-contained fireplace for resident safety. Access to the laundry room and garage is through a locked door for resident safety. LPA noted that the backyard has seating and shade for residents and visitors. The facility has battery operated smoke detectors in each room that are all working and the carbon monoxide detector is in the hallway and functioning normally. LPA observed a fire extinguisher near the kitchen that was in the green compression range, purchased on 4/17/2026. LPA noted that the facility has no obstructions in hallways, doorways or exits. LPA observed at least 2-days of perishable and at least 7-days of nonperishable foods for the current resident and staff. Medications are locked in a closet outside the laundry room. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records. LPA conducted a staff and resident file review.
Joselito and Licensee conducted a review of the annual care tool modules. There were no deficiencies cited at this time. LPA was unable to finish full care tool and may return at a later date.
Exit interview conducted with Licensee over the phone and Joselito, Licensee gave permission for Joselito to sign the report, and the report was provided to Joselito.
