Facility Evaluation Report

At 03:10pm, on 5/21/2025, Lice nsing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct a case management visit. LPA met with Licensee Janelyn Castaniaga, announced who he was and the reason for the visit.

On 5/13/2025 Community Care Licensing (CCL) received information that the facility licensee had issued eviction letters to residents in care due to the facility closing. On 5/14/2025 at 08:05am LPA conducted a phone interview with Licensee which revealed on 4/15/2025 Licensee issued eviction notices to three residents in care. On 5/14/2025 at 06:15pm Licensee emailed LPA a copy of the issued eviction letter which stated “This notice serves as a written notice notifying you that I, Licensee, Jane Castaniaga is downsizing to one care facility. Due to unforeseen personal matters, I’ve decided to only operate Villa gardens, other facility located in Atascadero, CALIF, owned and managed by myself.” Licensee stated to LPA she issued the eviction letters because she planned to close the facility. CCL was not provided a copy of this letter within 5 days of issuance as required. All three residents moved out of the facility as of 5/14/2025. LPA toured the facility and observed there to be no residents in care as of today's visit. Licensee was informed her facility is closed as of 5/21/2025. Facility license collected during the visit.
