Facility Evaluation Report

At 3:00pm on 12/17/2025, Licensing Program Analyst (LPA) Haner-Tomasko arrived unannounced to the facility to conduct a facility closure visit. LPA met with lead staff Holly Hobson announced who he was and the reason for the visit. LPA called Licensee Nancy Hobson over the phone and Nancy gave permission for Holly Hobson to sign the closure report.

LPA conducted a full tour of facility and facility property. LPA noted all client belongings were gone and no indication of clients residing in the facility. LPA collected the physical facility license. LPA noted the facility appeared to be absent of all clients at this visit and the lead staff indicated that the last time a client resided at the facility was 12/12/2025.
