Facility Evaluation Report

At 7:56am, on 10/15/2025, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA met with Lead Staff Holly Hobson, announced who he was and the reason for the visit.

Upon arrival to the facility at approximately 7:56am LPA was greeted by Person #1 (P1) who answered the door. LPA noted that P1 does not have a criminal record clearance to work or reside in the facility. Lead staff stated P1 is legally married to them, does not provide client care, resides part-time at the facility and is not cleared per California Health and Safety Code and Title 22 regulations.

Lead staff and LPA conducted a full tour of the facility. This facility has three client bedrooms (one is dual occupancy) and one full shared/public bathroom. There is a living room, a kitchen with dining space, and office bar space. Two live in staff bedrooms are located off the living room and one has a full bathroom; the other is for the Lead Staff's 5 children that live at the home as well. Access to the laundry room and pantry is through locked doors for client safety. LPA noted that the backyard and the front yard both have seating and shade for clients. LPA noted snacks in the kitchen for clients to enjoy freely. The facility has battery operated smoke detectors in each room that are all working, the carbon monoxide detector is in the kitchen and functioning normally. LPA observed a fire extinguisher near the kitchen that was tagged current and in the green compression range. LPA tested facility hot water at 116*(f), within regulation temperatures 105*-120* (f). LPA observed at least 2 - days of perishable and at least 7 - days of nonperishable foods.
