Complaint Investigation Report

On the allegation: Facility staff failed to report incident. LPA reviewed documentation of an incident report regarding R1 wandering away from the facility. The incident happened on 07/20/2020 and an incident report was sent to CCL on 07/24/2020. Based on the evidence this allegation is deemed Unsubstantiated at this time.

On the allegation: Food is poor in quality. LPA De Leon toured the kitchen and food supply which meets the food service requirements. Residents interviews revealed the food is of good quality but is not prepared and over cooked at times. The food was being served to residents rooms and was served cold which was cited on 29-AS-20200901154537. Based on the evidence this allegation is deemed Unsubstantiated at this time.
