Facility Evaluation Report
At 8:00am, on 5/14/2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA met with lead staff Debbie Pineda, announced who he was and the reason for the visit. At approximately 9:00am lead staff Vicky Hong arrived and participated in the visit.
Lead staff and LPA conducted a full tour of the facility. This facility is a two-story residential home and only the first floor is licensed by Community Care Licensing (CCL). The first floor has two dual occupancy client bedrooms, full shared/public bathroom, and the primary bedroom with en-suite bathroom is for live-in staff. There is a living room, dining space, kitchen and office area. Access to the laundry area and garage is through locked doors for client safety. LPA noted that the backyard and the front yard both have seating and shade for clients. LPA noted fresh fruit and snacks in the kitchen for clients to enjoy freely. The facility has battery operated smoke detectors in each room that are all working, the carbon monoxide detector is in the hallway and functioning normally. LPA observed a fire extinguisher near the front door that was tagged current and in the green compression range, serviced on 5/4/2026. LPA tested facility hot water at 116.8°F, within regulation temperatures 105-120°F. LPA observed at least 2-days of perishable and at least 7-days of nonperishable foods. LPA noted that the facility is clean and in good repair with no obstructions in hallways, doorways or exits. Medications are locked in a cabinet in the kitchen area. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records, finding no violations. Staff and client files are locked in the office area. LPA conducted a staff and client file review.
LPA and lead staff conducted a review of the annual care tool modules. There were no deficiencies cited at this time.
