Facility Evaluation Report
Licensing Program Analyst (LPA) Melisa Rankin along with Tri-Counties Regional Center (TCRC) Quality Assurance Specialist (QAS) Miquel Magana conducted an unannounced Case Management - Incident visit at the facility today due to a Special Incident Report (SIR) that was sent to TCRC. Upon arrival, there were two (2) staff, and two (2) clients present along with the Administrator, Marisa Cardona.
The purpose of today's visit is to follow up on a SIR pertaining to facility staff allegedly using verbal and physical force on Client #1 (C1). During today’s visit we interviewed the administrator going over the recent incident as well as anything that has led up to the cause of the recent incident. A review of chart notes was done, and copies will be provided by administrator. Guadalupe Ramirez, Quality Improvement (QI) will be doing an internal investigation and will provide TCRC and Community Care Licensing (CCL) a copy of the final report. No staff or client interviews were conducted at this time due to clients leaving to complete their plans for the day.
A follow-up visit will be done following QI’s investigation.
