Facility Evaluation Report
Licensing Program Analyst (LPA) Rankin arrived at 12:05 pm to conduct a 1-year annual visit to the facility above. LPA met with Administrator Shirlet Halliday and explained the purpose of the visit. Administrator had to leave around 2:05, final portion of the visit was done with a designee.
A tour of the inside and outside of the facility was conducted with the designee. The following was inspected and noted during the annual visit:
Physical Plant & Environmental Safety: The facility is 4 bedrooms and 2 1/2 bathroom facility currently occupies 4 Clients and employs 10 staff. The facility is clean, safe, and sanitary. The lighting and lamps are sufficient for the use of the facility and for Clients’ comfort. The facility kitchen is clean, safe, and sanitary. The showers have non-skid mats. The toilet, hand washing and bathing facilities are operational and secure grab bars are present. The pathways are clear of any obstructions. The facility has sufficient space inside and outside for activities and visiting. There are shaded areas in front and back for client use. The facility has a telephone and internet service for Clients’ use. The facility is operating in compliance with the granted fire clearance in regard to client usage. The facility has smoke and carbon monoxide detectors. Carbon Monoxide and smoke alarms were tested and working properly on the visit.
Staffing: The facility employes 10 staff and 1 Administrator. Staff records are kept confidential and locked in staff office. LPA reviewed 5 staff files. Files reviewed had current 1st Aid/CPR personnel Records/Application, Health screening with TB results, Criminal Record statements, and Fingerprint clearance. Administrator certificate expires 2/22/2026.
Personnel Records & Training: The facility keeps confidential files for each staff member. Staff files had annual training. All staff had current CPI training certifications.
