The inspector’s account

At 7:45am on 08/28/2025, Licensing Program Analyst (LPA) Jeffries arrived to the facility unannounced to conduct the facility annual inspection. LPA met with Administrator, Cherlyn Cradduck, announced who he is and the reason for the visit.

Administrator and LPA conducted a physical tour of the facility. This is a 3 bedroom, 2 bathroom, living room dining room, and kitchen facility, with a large fenced back yard and a larger open fenced front yard, There is table and chairs for seating and an umbrella for shade ample space for activities and visiting. Each room is double resident occupancy, one bath room is a shared by residents and the other bathroom is a suite bathroom located in room 1. LPA observed bathrooms to have liquid soap, paper towels, and non-slip bath mats. Water was tested throughout the facility and all temperatures tested within regulations range of 105*-120*(f). LPA noted that all bedrooms had proper linin, storage and with chairs and night stands. LPA noted that the medication closet is located in the hall way and was locked and secured. LPA noted that the 1st aide kit and fire extinguisher were located in the medication closest both meeting regulations requirements. LPA observed at least two days of perishable foods and seven days on non-perishable foods on hand for 5 residents and staff. LPA observed at least 72 hours of emergency water located in the facility garage. LPA reviewed facility emergency disaster plan, infection control plan, facility plan of operations, centrally stored medication records(CSMR), staff files including but not limited to training, and all resident files. LPA noted that the facility was clean and in good repair and all exits were clear and free of obstruction. LPA noted that there were no violations or citations issued as a result of this facility's physical annual inspection.

Administrator and LPA conducted a full review of the annual care tools modules and noted no violations or citations. At time there were no violations or citations issued as a result of the full facility annual inspection.
