Facility Evaluation Report

At 8:00am on 03/26/2026, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct a final facility closure visit.

On 01/12/2026, Licensee made contact to Community Care Licensing (CCL), LPA Jeffries of intention to close facility due to cost of operations. At that time LPA requested eviction letters and advised Licensee on Resident transitions. On 03/23/2026, Licensee emailed LPA to inform CCL that the last resident at the facility had moved out. On 03/26/2026, LPA conducted a physical inspection of the facility. LPA noted that he observed no residents, and no evidence of residents currently residing at the facility. Licensee surrendered facility license to LPA. Licensee noted that the last resident left the facility on 03/23/2026. LPA thanked Mr. Castillo and concluded the closure visit.
