Facility Evaluation Report
At 8:00am on 05/12/2025, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct the facility annual inspection. LPA met with Administrator, Joe Castillo announced who he is and the reason for the visit.
This is a 5 bedroom, two bathroom facility with 2 bedrooms as single resident occupancy, two bedrooms as double resident occupancy, and one bedroom reserved for staff. One large bathroom for clients and a staff bathroom off the staff designated room. There is a living room, kitchen, and dining room, there is a large back porch and yard with awning with table and chairs for outdoor use. LPA noted that a section of the backyard is currently under construction, adding artificial grass, fencing and railings. Licensee stated completion of the renovations by August of 2025. LPA noted that there is a first aide kit stored in a locked cabinet in the kitchen where the medications are locked and stored. LPA observed all resident rooms to meet regulation standards with furniture and linins. LPA noted that bathroom has paper towels and liquid soap. LPA observed at least two days of perishable foods and at lease seven days of non perishable foods on hand for residents and staff. LPA noted that the facility was clean and in good repair. LPA observed two fire extinguisher to be primed and in the green. LPA observed and tested facility carbon monoxide detector. LPA noted that the smoke detector system is hardwired and requested Licensee to video document a test of working and functional by 05/19/2025 and send verification to LPA's cell phone (information provided). LPA noted that the hallways and exits were free and clear of debris.
LPA reviewed Emergency Disaster Plan, Infection Control Plan, Resident and Staff files, and Administrators Certificate. Licensee and LPA conducted a full review of the annual care tools modules and noted no citations issued. LPA noted that no citations were issued as a result of this annual facility inspection.
