The inspector’s account

Licensing Program Analyst (LPA) De Leon conducted a case management continuation annual. LPA met with Raniele Jeffers, Administrator and explained the purpose of the visit.

LPA conducted a file review for staff and clients.

Staffing : LPA reviewed 6 staff files for required records, staff had applications, health records with TB results, Finger print background clearances, and valid 1st Aid/CPR certificates. Administrator File had continuing education.

Personnel Records & Training: Staff training files reviewed and all staff 8 plus hours of training on file for 2024-2025.

Clients Records & Incident Reports: LPA reviewed 5 residents files for required records, signed Admission Agreements, Medical Assessments with TB results, consent forms, Personal rights, ID and Emergency Form, and Appraisal Needs and Services Plan.

Exit interview conducted and copy of report printed for Administrator.
