Complaint Investigation Report

At 10:22 am on 3/30/22, LPA spoke with Administrator Debbie Bertrando. Administrator stated that they do not write incident reports for independent time, only for elopements. She states the last elopement was in March 2021 and this was reported to CCL.

On 3/30/22 at 10:25 am, LPA spoke with Crisis Supervisor Paola Morales. Ms. Morales states that there are only two residents with a tendency to elope and they have not eloped in over a year. She states that independent time is earned by R1 and R2, and explains that in independent time, staff drop off the resident at their place of choosing, and resident calls staff to pick them up. She states there have been no incidents of elopement during independent times.

On 3/30/22 at 8:20 am, LPA reviewed incident reports from January 2021 to present. There is one incident report for R1 from 3/29/21. There were no incident reports for independent time.

Based on the evidence obtained, the allegation “Facility is falsifying incident reports” is deemed Unsubstantiated. The facility does not report independent times for residents in incident reports to CCL which is valid, and elopements are being reported to CCL.
