Facility Evaluation Report

Licensing Program Analyst (LPA) De Leon conducted a Case Management Annual Continuation visit. LPA met with Erin Tidmarsh, Human Resources Director of Options and explained the purpose of the visit.

LPA visited the Human Resource office in San Luis Obispo to review staff files and training to complete the annual review.

Staffing: The facility employes 7 staff and 1 Administrator. Staff records are kept confidential. LPA reviewed 6 staff files for 1st AID/CPR, Finger print clearances, Applications, Health exam with TB results, and Administrator Certificate expires 05/19/2026 and Administrator file is up to date.

Personnel Records & Training: The facility keeps confidential files for each staff member. 6 staff had current 1st AID/CPR, records were present, up to date and kept confidential. Initial and Annual Training requirements are being met for 2025. All staff files had 8 plus hours of Annual training on file.

Exit interview conducted and copy of report printed for Administrator.
