The inspector’s account

On 11/18/22 at 11:47 am, Licensing Program Analyst (LPA) Chavez conducted an unannounced on-site annual infection control visit to the facility above. LPA met with Jacob Harrington, Site Director, and explained the purpose of the visit.

LPA toured the facility with the site director and observed the following: The facility has infection control signage near the front door. LPA suggested placing on the front door to be more visible and to add the visitor policy to the front door. Administrator will move and post signage, take a photo, and send to LPA by 11/21/22. The facility has signage throughout the facility on handwashing. Staff are wearing masks. The facility has soap dispensers and paper towels dispensers in client bathrooms. Fire extinguishers (2) are located in the “big room” and the foyer. The extinguishers are fully charged and were inspected on 2/23/22. LPA observed a cabinet under the kitchen sink that contained cleaning products and was locked, however, the lock still allows the doors to open approximately 8 inches. Administrator will change the lock to ensure these items are inaccessible, take a video and send to LPA by 11/21/22. LPA observed an area on the north side of the backyard where there is a gap of approximately 15 inches between the ground and the bottom of the fence. This is a potential safety hazard. Administrator will secure the area, take a photo, and send to LPA by 11/28/22.

At 12:15 pm, LPA conducted the Infection Control mitigation module with the site director. No deficiencies cited.
