Facility Evaluation Report

Licensing Program Analyst (LPA) De Leon conducted a case management other visit to the facility above.

LPA met with Administrator Joni Chapman and explained the purpose of the visit.

LPA is making a quarterly visit to check on the residents, staffing and the physical plant for compliance.

LPA toured the facility with Administrator, checked the food supply which is within regulation, the facility is clean, safe and sanitary, emergency supply of food and water is present, medications are locked in a room, washer and dryer were being used and working on the visit.

All 4 residents were home, 1 was in the shower, 1 was in the kitchen and the other 2 residents were in the living room.

LPA observed the Community Care Licensing Accusation posted in the dining room. Administrator stated all residents and responsible parties were also notified.

Exit interview conducted and copy of reported printed for Administrator.
