The inspector’s account

Licensing Program Analyst (LPA) De Leon conducted a case management deficiency visit to the facility above. LPA met with Joni Chapman Administrator and explained the purpose of the visit.

During the complaint investigations, LPA learned that R1 has 1 on 1 caregiver assigned. Tri-Counties Regional Center (TCRC) clients are the residents living in the C.A.L.L. Facility. R1 is to be provided with a 1 on 1 caregiver on a daily basis. According to the facility schedules the facility has been short staffed on several occasions.

LPA reviewed the facility staff schedules for July and August 2025 which revealed a good amount of shifts were left blank and not covered by staffing. Dates with blank spots not filled in the schedule and not covered by another staff were July 4th, 5th, 6th, 10th, 12th, 13th, 17th, 18th, 19th, 20th, 22nd, 24th, 25th, 26th, 27th and 29th. Dates with blank spots not filled in the schedule for August were the 1st, 2nd, 3rd, 4th, 5th, 7th, 8th, 9th, 10th, 12th, 14th, 15th, 16th, 17th, 18th, 19th, 20th, 21st, 22nd, 23rd, 24th, 25th, 26th, 28th, 29th, 30th and 31st.

Additional staffing is needed at the facility, the facility is currently short staffed and is not meeting the 1 on 1 care requirement with R1 during the required hours.
