Facility Evaluation Report

Licensing Program Analyst (LPA) De Leon conducted a Case Management Incident Report visit to the facility. LPA met with Joni Chapman and explained the purpose of the visit.

CCL received a self reported incident report on 07/21/2025 and an amended incident report on 07/22/2025 regarding a staff pushing a resident several times to get Resident (R1) to go out the door.

The facility placed staff on administrative leave pending internal investigation. The video surveillance footage was pulled, watched and a copy of video was sent to CCL. At one point Staff 1 (S1) used S1’s hand to push Resident 1 (R1) towards the door, later in the video S1 is seen using both hands and guiding R1 to go outside.

At no time are the staff allowed to push a resident. the facility is a hands off facility and is trained in CPI but does not use restraint or seclusion with any residents in care.
