Facility Evaluation Report

Licensing Program Analyst (LPA) De Leon conducted a case management visit to the facility above. LPA met with Joni Chapman and explained the purpose of the visit.

Community Care Licensing (CCL) received late incident reporting on 2 separate incidents.

One incident occurred with a resident falling over hitting face and getting a black eye on 02/08/2025 but was not reported until LPA De Leon asked the facility to make an incident report, CCL received late incident reporting on 02/25/2025.

Another incident occurred on 06/11/2025 where staff locked a resident outside while doing medication pass for other residents in care it was reported late to CCL on 07/18/2025.

Exit interview completed, deficiency cited, copy of report and appeal rights printed for administrator.
