Licensing Program Analyst (LPA) De Leon arrived on 11/26/2025 for an unannounced inspection to follow up on a citation related to an incident report investigation. The LPA met with Administrator Joni Chapman and explained the purpose of the visit.
On January 29, 2025, the Department concluded an incident report investigation regarding the following incident: Staff #1 (S1) physically abused Client #1 (C1) while in care.
The licensee was cited for California Code of Regulations (CCR) 80072(a)(3) Personal Rights.
At the time of the case management visit on January 29, 2025, an immediate civil penalty of $500 was issued. Due to the fact that this was a repeated violation, an additional $500 was levied. The licensee was informed that an additional civil penalty might be assessed based on Health and Safety Code §1548(f)(1)(A).
The Department has concluded an analysis and has determined that a civil penalty is warranted for physical abuse. The Welfare and Institutions Code Section 15610.63 for physical abuse means any of the following: Battery, as defined in Section 242 of the Penal Code. This is evidenced by the facility surveillance video capturing S1 committing violence against C1 by pinning C1 against the wall with the kitchen gate. C1 suffered a fractured leg which required hospitalization and surgery.
Today, 11/26/2025, the Department will be issuing a civil penalty per Health and Safety Code §1548(f)(1)(A) for a violation that the Department constitutes as physical abuse in the amount of $10,000. However, since an immediate civil penalty of $500 was previously issued on January 29, 2025, the amount of the civil penalty issued today will be $9,500. Continued 809-C
