Complaint Investigation Report
On the allegation: Staff does not ensure the facility is stocked with food. LPA interviewed 4/5 staff which revealed the facility always has food for 3 and 2 snacks daily for 4 clients, there is always food to make meals, occasionally an item or two may run low but grocery shopping is done weekly so the clients never go without. 1 of 5 Staff reported that a staff was taking or throwing away food to make it look like the newly hired House Supervisor was not doing their job. 4/5 staff agreed that the lead staff was making it hard on the supervisor to learn and do the job but it never led to the clients doing without anything. LPA toured the facility on 01/24/2024, 02/20/2024 and 04/11/2024 the food was well stocked with extra supplies of food and water for emergency purposes; therefore, this allegation is Unsubstantiated at this time.
On the allegation: Staff inappropriately restrained a client. LPA interviewed 4/6 staff which did not reveal anyone inappropriately restraining a client it care. 1/5 staff said the facility no longer has a safe room, that along time ago they had a client in care that they needed to use it with but no longer had the client in care and since then the room was turned into staff office and medication room. 1/5 Facility staff did not have current CPI Training, all other staff was current. The facility did not have any incident report for client being inappropriately restrained. 4/5 staff felt the House Supervisor was kind, caring and good with the clients and never seen anything to indicate a client had been inappropriately restrained. 4/5 staff stated the lead staff did not want the house supervisor to get the job and was making the job harder for the house supervisor. The house supervisor resigned on 02/20/2024. The staff felt the clients were always cared for even with the staffing issues going on therefore this allegation is deemed Unsubstantiated at this time.
On the allegation: Staff is mistreating the clients while in care. LPA interviewed 4/6 staff which revealed around the time of the complaint the program had changed many administrators, house supervisors and lead staff positions through several of the facilities which led to the lead staff being upset about the House Supervisor chosen for the position at the facility. The lead staff was not helping the house supervisor and tension was felt with the staffing. 4/6 staff stated the House Supervisor was kind, caring and good to the clients in care and never mistreated them in anyway. 5/6 staff said the house manager resigned from the position due to the lead staff and the lead staff was moved over to another facility to work. 4/6 staff felt this was a false allegation and was don’t occurring to any of the clients in care therefore, this allegation is deemed Unsubstantiated at this time.
