Facility Evaluation Report

Licensing Program Analyst (LPA) De Leon conducted a case management incident visit to the facility above. LPA met with Administrator and explained the purpose of the visit.

LPA requested a staff roster, resident roster, Resident 1's Medication list and a copy of R1's Cross System Crisis Prevention and Intervention plan dated 05/25/2026 and revised plan dated 06/17/2026.

LPA conducted a welfare check on all four residents in care.

Resident 1 (R1) has been displaying aggressive behaviors, hitting staff, residents and visitors in the facility.

The facility is working closely with Tri-Counties Regional Center (TCRC), R1's Doctors, Psychiatrist, Crisis Support and currently has a 2:1 ratio of staffing for R1 to keep R1 and other 3 residents safe.

LPA spoke with all 4 residents in care and at this current time all for residents are doing well.
