Facility Evaluation Report

Licensing Program Analyst (LPA) De Leon conducted a 1 year infection control annual visit to the facility above. LPA met with Todd Hove and explained the purpose the the visit.

LPA took a physical plant tour and did not observe any clients in care. Administrator stated the program is still running on-line services with clients only, On-line census is 21.

The program is running Zoom Meetings and prepares packets for drop of and pick up to clients. No in-person services are taking place. There are 2 full time staff and 1 part time staff currently working out of the Spruce location.

The Facility has full PPE available. Covid signs are posted through-out the facility. Restrooms are fully stocked and have hand washing signs. Masks and Hand Sanitizer is available for all staff.

Administrator will contact LPA when in-person services resume.

Exit interview conducted and copy of report emailed to Administrator.
