The inspector’s account

Licensing Program Analyst (LPA) De Leon conducted an unannounced Case Management Closure Visit to the facility above. LPA met with Licensee/Administrator Connie Ray and explained the purpose of the visit.

Licensee has decided to retire and sell the property. Licensee sent into CCL a copy of each 60 day notice to each resident and responsible party. As of 07/01/2025 all residents have been placed and the facility is closed and no longer operating. Licensee has sent in the license to CCL.

LPA toured the inside and outside of the facility. The facility is empty with no residents in care. No staff was present. The home has sold and currently in escrow to close towards the end of the month.

Exit interview and copy of report sent to Licensee/Administrator.
