Facility Evaluation Report

On 10/13/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA met with Facility Designated Administrator (FDA), Maybelyn Magsayo-Ugale and explained the purpose of the visit. The purpose of the visit was to conduct an annual visit.

It was learned that this facility currently has undergone the vendorization process through Valley Mountain Regional Center to accept and retain Level 6 residents who are 60 and above.

LPA Pascua obtained a 60-day notice in May 2025. LPA Pascua obtained a new Plan of Operation to reflect the facility changes prior to the vendorization change.

Current census was 0. The FDA has a current and active administrator certificate #7008216740 and expires on 09/17/2027.

A tour of the facility was conducted to ensure that facility safety was in compliance, such as the fire extinguishers, smoke detectors, and carbon monoxide were present. All were present and in good repair at this time. A tour of facility bedrooms were conducted to verify that there are currently no residents in care.

LPA advised FDA Ugale to notify the department once facility obtains their first resident.

LPA Pascua asked for an updated facility sketch to reflect new bedroom changes.

Based on the observations made, there are no deficiencies observed during this annual visit.

An exit interview was conducted and a copy of this report was provided to the facility.
