Facility Evaluation Report

On 04/29/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unanounced to this facility to conduct an annual visit. LPA met with Facility Designated Representative (FDR), Christina Goforth and explained the purpose of the visit. The purpose of this visit was to conduct an annual visit.

Current census was 112. A brief interview with FDR Goforth was conducted.

This is facility is licensed for 180 residents, 137 of whom may be non-ambulatory, 31 ambulatory, and 12 bedridden residents. This facility also has a hospice waiver for 15 residents and has a dementia plan on file.

During the course of this visit, LPA reviewed facility records including but not limited to 3 resident files and 3 staff files. All of which were complete and up to date.

Due to insufficient time, LPA will come at a later date to complete the annual inspection

As a result of this visit, no deficiencies were observed or cited during this annual visit. An exit interview was conducted and copy of the 809 was provided to Facility Designated Representative.
