On 10/16/2023, Licensing Program Analyst (LPA) Arielle Pascua arrived announced to this facility to conduct a case management visit. LPA met with Facility Designated Administrator, Priya Lal and explained the purpose of the visit.
Current Census was 121. An interview with FDA Lal was conducted.
The purpose of this visit was response to a phone call that LPA Pascua received this morning regarding R1.
LPA will obtain documentation regarding R1 via email.
No deficiencies cited during this case management visit, exit interview was conducted.
