Facility Evaluation Report
On 07/14/26, Licensing Program Analyst, LPA, Melina Oropeza arrived unannounced, to conduct a quarterly compliance visit related to the concerns about weaponizable objects entering the home. The LPA met with the administrator Kathleen Quiaot and explained the purpose of the visit. The facility is licensed for 6 as a a Adult Residential Facility for females aged 18-59. The Current census is 4 ambulatory individuals, no restricted conditions. 3 of the residents were in Day Program and 1 resident was sleeping.
LPA Oropeza inspected the physical plant, including but not limited to: resident bathrooms, resident bedrooms, hallways, evacuation routes, common areas such as the kitchen and recreation zone, and the facility exterior. The required furniture and furnishings were found to be present and in good repair. The facility is noted to be clean, with no obstructed or poorly lit traffic areas.
The LPA reviewed the weaponizable objects log, the most recent training and interviewed the staff who described a process that respects the voluntary nature of the bag checks and increases monitoring when bag checks are refused.
In regards to staff training, LPA Oropeza reviewed the last training certificates for ARF Laws and Regulations on 03/19/26 . LPA Oropeza also reviewed administrator's training certificates for Meaningful and Measurable Goals on 03/11/26 and Prevention and treatment of Suicide on 01/30/26.
No concerns about compliance at this time. No citations were issued as part of this visit, an exit interview was conducted, and a copy of the report was read and given to the Administrator, Kathleen Quiaot.
