Facility Evaluation Report
on 8/6/2025 at 12:20 pm Licensing Program Analyst Noel Wolf Petersen arrived unannounced to conduct the quarterly compliance check related to the concerns about weaponizable objects entering the home. The LPA met with the administrator Kathleen Quiaot and explained the pupose of the visit. Willow care home is licensed for 6 as a a Adult Residential Facility for females aged 18-59. the Current census is 4 ambulatory individuals, no restricted conditions.
A Physical Plant inspection was conducted including the staff room, bed rooms, client bathrooms, common areas, storage areas, kitchen, exteriors and evacuation route. The facility is clean, and the traffic areas are unobstructed and well lit. Required postings are present and of the correct dimension exepting the federal work requirements poster. The administrator will order one. LPA Wolf Petersen noted some Laundry detergent in the staff room which was not off limits to the residents, the administrator provided that none of the residents 602's designate an issue with doing thier own laundry. The staff room(master bedroom) is a lockable room, and residents could easily be barred from access to the laundry detergent if a concern developed.
The log of door check time exists, and the LPA observed two clients coming home from program and getting both physicaly looked over and non-intrusively being asked to check the lunchboxes and backpacks and pocket contents of the clients.
The staff training designated in the concerns of potential risks to clients in care by the expressed behaviors has signed completion certificates in the staff binders.
No citations were issued as part of this visit, an exit interview was conducted, and a copy of the report was read and given to the Administrator
