Facility Evaluation Report

LPA Lewis and Oropeza arrived unannounced at the facility and met administrator to conduct a case management visit regarding an incidents that took place on 03/07/2026.

R1 signed out to go on a walk and did not return. AWOLING from from the facility.

LPA'S conducted a case management inspection to ensure Title 22 compliance and health and safety concerns. An interview was conducted with the administrator and the facility was toured .LPA'S also reviewed resident records to check for completeness, all necessary documents were in place.

Based on inspection, and documentation reviewed during this case management, the department has closed this case management and no further investigation is required R1 is able to leave the facility unassisted. No deficiencies were cited as a result of today’s visit.

An exit interview was conducted and a copy of this report was left.
