Facility Evaluation Report

On 9/11/25 at 12:45pm Licensing Program Analyst(LPA) Noel Wolf Petersen, arrived unannounced to conduct an annual inspection, LPA met licensee/administrator Maria Teresa Orpilla to explain the purpose of the visit.Weston Ranch Care Home is a 3 bedroom facility for 6 adults age 18-59 with a current census of 6, no restricted conditions amongst the folks currently there.

physical plant was inspected including but not limited to: kitchen, common areas, bathrooms, staff room, bedrooms, exteriors, and evacuation routes. the facility is clean, traffic areas are well lit and unobstructed. kitchen had lockable storage for knives, medication, and toxics. common areas and exteriors had space for activities and furniture in good repair. Bathrooms had funtional hardware and delivered water at 111*F. bedrooms had required furniture and furnishings. Exteriors had some excessive spider webbing, LPA gave guideance that the facility should get the spider tool and take care of those about once every two months. facility was receptive. evacuation route gate swings freely and latches closed. LPA Observed some medication that was prepoured out to the next two med pass services, LPA cautioned not to do that anymore. Citation issued on following D page. Fire Smoke alarm was not checked, LPA asked licensee to check on that when Tv time was over. fire extinguisher is dated June 6 2025.

5 client records were reviewed, including but not limited to: needs and appraisals, signed admission agreements, most recent medical assessments, and preadmission appraisals.

5 staff records were reviewed, including but not limited to: fingerprint clearance with DoJ/FBI, Cpr/first aid certification, and ongoing training.

administrator file was reviewed, including but not limited to: program design, house rules, infection control plan, surety bond, liability insurance, administrator certification. facility plan with sketch, LPA is going to look for the 850 in the physical record storage.

2 client interviews were conducted, 2 staff interviews were conducted.

Appeal rights provided, an exit interview was given, a copy of the report was read and given to the administrator.
