The inspector’s account

On 6/10/25 at 1:30 pm, Licensing Program Analyst(LPA) Noel Wolf Petersen arrived unanounced to conduct an annual inspection and met with administrator Joseph Nimene and explained the purpose of the visit. Walters Residential Home Care 3 is a adult residential for 4 clients with a current census of 4 ambulatory clients.

Physical plant inspection was conducted, including but not limited to the bedrooms, bathrooms, common areas, hallways, exterior and evacuation routes. The facility is clean excepting a wrecked mattress outside and traffic areas are unobstructed. The administrator is renting a truck to remove the trash immediately. The fire extinguisher is dated 5/6/24, out of date, the administrator is taking the fire extinguisher to the lodi fire department to exchange it immediately. The water temperature is measured 109 on the first floor bathroom, 78 on the second floor. The administrator raised water temperature slightly to try and deliver water to the second floor at a higher temperature without going over on the first floor.Required poster for federal work requirements was not present, ombudsman poster was of the incorrect dimensions. The administator ordered new posters. The sharps and medications were locked away, but several toxics were left out including 2 hand soaps, the administrator locked the chemicals away when they were pointed out. The anti bug encasements were around the beds. The nightlights for the bathrooms and hallways were not present. The administrator ordered night lights. the trash cans in bathrooms and kitchens did have lids. The first aid kit has all the required items. The fire alarm is functional, fire drills conducted quarterly. 2 days of perishable and 7 days of non perishable food is present in the facility. Menus and activity logs correspond to what is offered to the clients, the clients can make changes to offerings in monthly meetings. The Evacuation gate should be rehung, it binds a little bit and could prevent easy exit. the administrator rehung the gate. no bodies of water were observed.
