Facility Evaluation Report

Licensing Program Analysts (LPA) Kesha Lewis arrived at the facility unannounced for the purpose of conducting a case management incident inspection regarding incident reports received dated 3/17/2025 for incident that happened in March 03/15/2025 and is ongoing. LPA explained purpose of visit to the administrator.

The incident reports were regarding R1 being sent to the hospital for a foot issue then then another time being sent out again for a fall that resulted in a broken femur. LPA reviewed incidents reports Resident records. Resident has moved to a skilled nursing facility on 04/22/2025.

Administrator was able to provide addition needs and services plan for R1 that covered the in between period of 3/15-/4/02/2025.
